1. KEZI-9 "Click N Win" Contest is a monthly on-line contest where participants must complete an on-line survey in order to enter the contest.
2. Each participant will qualify for a chance to win an instant prize. During months where a grand prize is offered, in addition to an instant prize, each participant will qualify for a chance to win the grand prize, which will be given away at the end of every calendar month. One grand prize winner will be randomly selected at the conclusion of each calendar month that a grand prize is offered.
Grand Prize for May Click and Win promotion is:. 2 nights lodging, champagne and flowers in room, a $50 dining certificate and a spa treatment, value $750 Prize value not good for cash, vouchers are non refundable or non transferable.
3. Each calendar month, Ten (10) participants will be randomly selected as "instant winners." This months instant win prize is a gift certificate to Abby’s Pizza value $20. These winners will be notified of their win upon completion of their survey via a website pop up message. Each instant winner will be randomly selected. All contest participants will be eligible to win the grand prize during months in which a grand prize is offered.
4. To be eligible to play, all qualifiers must meet the following requirements:
• Players must provide their name, phone number and valid email address.
• Players must be at least 21 years old.
• All contestants must be residents of the State of Oregon
• Players must hold a valid driver’s license issued by the State of Oregon.
• Players must be United States citizens or legal residents.
• Limit one qualifier/player per household within thirty (30) days of the start of each contest.
• No purchase necessary. Contests are subject to federal, state and local laws. Void where prohibited by law.
• All entrants must have an e-mail account.
• Prize is non-transferable. Prize has no cash equivalent. Prize may not be exchanged.
• KEZI-9 will be the sole and final judge of eligibility and winner verification.
• Winner’s name, voice, and likeness may be used for publicity, promotion and/or news purposes without further compensation.
• By acceptance of prize, winner assumes full responsibility for all applicable income taxes, sales taxes, license, and other fees that may apply.
5. All entries become property of KEZI-9 and will not be returned. False or deceptive entries or acts will render entrants ineligible.
6. Viewers understand that if they enter any KEZI-9 contest (mail-in, web site or on-air) they may be contacted by KEZI-9 and videotaped for on-the-air use on KEZI-9. Viewers further understand that permission to use their voice, likeness and/or name is granted by entering to win or calling-in to win any contest on KEZI-9. Each winner further grants KEZI-9, its sponsors and affiliates full permission and authority to use their name, likeness and/or voice in all forms of advertising without further compensation or permission.
7. Any viewer requesting contest rules or a list of winner names must send a self-addressed envelope with a request for contest rules or winner names to: PO Box 7009, Eugene, OR 97402.
8. KEZI-9 is not responsible if a contest sponsor pulls out of a contest for reasons beyond KEZI-9 control. KEZI-9 is not responsible for continuing the contest and may substitute prizes to fulfill obligations.
9. Each winner of a prize will need to come to KEZI-9 (2975 Chad Drive, Eugene, OR 97402) provide proof of identification (i.e.: valid Oregon State driver’s license, picture identification card) AND a social security number before any prize is awarded. Prizes must be accepted as awarded and are not redeemable for credit, are non-transferable, non-exchangeable and are non-endorsable. In the event a substitution for a prize may become necessary due to unavailability or circumstances beyond KEZI-9 control, a reasonable prize will be substituted at the sole discretion of KEZI-9 and any/all contest sponsors. KEZI-9 is not responsible for replacing tickets from cancelled shows/events. All prizes must be picked up within two weeks of winning unless previous arrangements have been made with the KEZI-9 Promotions staff. If a prize is not picked up within two weeks, that prize is forfeited.
10. Winners not able to come to the KEZI-9 offices between 8:30am and 5:30pm, Monday through Friday, may choose to have someone else pick up their prize. The winner will be faxed, mailed or e-mailed a copy of the 1099 Winner Release Form (& travel companion form if applicable) and a prize release form. The winner must fax or mail back both forms along with a copy of his/her driver’s license and social security card before prize will be released to a third party. Alternatively, these forms can also be sent with the third party when they pick up the prizes. The third party must bring photo identification with him/her before the prize will be released to the third party. The winner (or winner’s guardian) agrees to pay all expenses, taxes & fees not included in the prize.
11. Employees of KEZI-9, and any other employee of Chambers Communications, participating vendors, affiliates, their dependents and immediate families, as well as employees of contest sponsors, and employees of other television stations are not eligible to participate or win.
12. Participants waive any and all claims against KEZI-9 and the contest sponsors and their affiliated companies and employees for injuries, damages or losses related to the contest.
13. KEZI-9 reserves the right to change and/or amend the contest dates and rules at any time.
14. Final determination of all winners will be determined solely by KEZI-9.
15. By entering, participants agree to be bound by these official rules and the decisions of the sponsor, which shall be final in all matters pertaining to the contest. The sponsor may deem an entrant ineligible to participate in a contest if the entrant disregards the official rules.
16. The sponsor, its officers, directors, agents, and employees are not responsible for and shall not be liable for: (1) garbled, illegible, incomplete, damaged, misdirected, late, lost, or failed entries; (2) telephone, electronic, decoder, hardware or software program, network, Internet, or computer malfunctions, failures or difficulties; (3) any condition caused by events beyond the control of the sponsor which may cause the contest to be disrupted or corrupted; (4) any losses or damages of any kind caused by the prize or resulting from acceptance or use of the prize, or from participation in the contest; (5) any printing or typographical errors in any materials associated with the promotion; or (6) use of person's name, likeness, or voice. The sponsor reserves the right, in its sole and absolute discretion, to suspend or cancel the promotion should virus, bugs, or other causes beyond the control of the sponsor corrupt the administration, security, or proper play of the offering. The sponsor further reserves the right to create a database to utilize the name, street address and e-mail address of an entrant for marketing or other purposes. Entrants may opt out of this database by calling or by mailing their request to Click N Win Contest, c/o KEZI-9 2975 Chad Dr, Eugene, OR 97402. Oregon law shall apply to this promotion. Any claims must be resolved in the State of Oregon.

18. By participating in the contest, each participant agrees to release and hold the Sponsor and the employees, officers, directors, shareholders, agents, representatives of the Sponsor, its parent companies, affiliates, subsidiaries, advertising, promotion, and fulfillment agencies, and legal advisors, harmless from any and all losses, damages, rights, claims and actions of any kind in connection with the contest or resulting from acceptance, possession, or use of any prize, including without limitation, personal injury, death, and property damage, and claims based on publicity rights, defamation, or invasion of privacy. Any use of robotic, automatic, programmed or similar entry methods will void all such entries by such methods.
19. Winner certifies he or she has read, understands and will abide by all contest rules.
